1. Fill out the form below and click ‘submit’ to request a gift card.
2. Once we’ve received your request, we’ll send you a payment request.
3. Send an email to hello@urban-goddess.com after you’ve completed your payment, so we can finalize your request.
4. We will then email you the Urban Goddess gift card, so you can print it out (or give it digitally) to your recipient!
